Professional attendance by a prescribed medical practitioner, in relation to a mental disorder, lasting at least 20 minutes and involving:(a) taking relevant history and identifying the presenting problem (to the extent not previously recorded); and(b) providing treatment and advice; and(c) if appropriate, referral for other services or treatments; and(d) documenting the outcomes of the consultation
